The Join app is an excellent tool for comprehensive stroke centers, especially those serving the community utilizing a hub-spoke model of care. The company is extremely responsive to local needs and rapidly adapts the software and develops new applications based on clinician input.
I have used the Join solution realizing the dual purpose of stroke care and clinical research with its effective team collaboration while working as a Vascular Neurology Fellow. Its simplicity, making clinical data available in real-time, and integration into the large healthcare system where I worked was wonderful for our team of clinicians and researchers. I aim to integrate this application at my comprehensive stroke center going forward as well.
I have used Join solutions for over three years now. JoinTriage has had a significant and positive impact in the care of stroke patients by streamlining multidisciplinary communication, improving triage and decreasing time to treat.
The Join and JoinTriage apps are mHealth solutions that are transforming the way clinicians and caregivers are communicating and sharing information securely for acute care coordination and collaboration across health system. It provides support along the patient care continuum, from EMS ambulance to nearest ED center, and from a primary hospital to comprehensive hospital, in a seamless cloud environment coupled with administrative dashboard. I believe that expanding such real-time communication, connected care, proper data handoff and smooth experience to post-acute care patients would be an additional benefit.
I have been successfully using the Join app over several years for acute stroke care including interventions at comprehensive stroke centers and primary stroke centers as well as remote consulting to triage and transport the patients in the health system and the region across neighboring states for better patient outcomes. The Join medical communications app along with its built-in features of protocol timestamping, telemedicine and DICOM viewer helps with efficient care coordination, collaboration and decision making for time-critical stroke care. Its companion app, JoinTriage, for EMS ambulances is helping first-responders to quickly conduct stroke assessment, identify the appropriate hospital and pre-notify to the destination care team. We’re seeing the crew in transport vehicles becoming an extension of our care team for better preparation and quality care delivery each time 24×7.
The Join app solution is an important advance for providing care for acute stroke patients. It allows physicians and other healthcare personnel to interact with patients and each other remotely in a secure and easily accessible manner.
We have collaborated with Allm to create a custom FAST-ED scale in JoinTriage for West Virginia use by capturing tPA potential and functional status for better patient outcomes. The innovative mobile apps of JoinTriage and Join facilitate the total coordination, real-time communications, team collaboration, sharing information including medical imaging studies, accessing case list, protocol timestamping, telehealth, and seamless clinical research for a better state-wide system of care. We hope to continue to collaborate as these convenient and powerful tools are implemented throughout West Virginia.

A 15-hospital system recently implemented Join to improve care coordination throughout its network. To date, this system has successfully used Join as telehealth and care coordination tool for cardiac and brain aneurysm patients. Join has enabled collaboration among experts in this integrated delivery network, improving the quality of care and boosting patient outcomes.
Another integrated delivery network made up of 10 hospitals successfully implemented Join to enable network collaboration among clinicians. The key to success in this system was the ability to seamlessly share DICOM images through direct integration with the PACS. Along with improving patient outcomes, this Integrated Delivery Network (IDN) has experienced a reduction in clinician burnout by providing a simple, seamless platform for communication.

ED physicians conducting the research needed a quick and simple way to store and share assessment information, patient data, and intake forms. They decided to use Join as the central communication hub because it is already integrated with JoinTriage and makes communication of all types of information fast and seamless.
Assessments performed by EMTs were compared to assessments performed on the same patient in the ED; results were recorded and communicated through Join.
As a result of this research, JoinTriage became part of the state-wide stroke assessment protocol, and the hospital opted to expand Join usage into neurology care. This project has connected remote facilities and EMS agencies to more comprehensive centers, increasing patient access to care.

Due to the high volume of medical facilities in this area, ambulances have many emergency departments to choose from. Often, the capabilities of the centers they chose did not align with the needs of stroke patients. The CSC often received transfers from other facilities when the better option was to bring the patient there in the first place. This region needed a tool that would provide EMS guidance by matching stroke patients to the most appropriate facility.
The use of JoinTriage as a stroke assessment tool in the field is standard for all ambulances, and upon assessment results are instantly sent to the ED through the Join Platform. Through the Join application, EMS workers are able to receive feedback and patient updates from the ED. They are able to use this feedback to improve practices.
Integration has improved the quality of care and streamlined transport and ED admission processes for stroke patients. The facility experienced a complete return on investment in the first week of use through improved routing capabilities. Additionally, the relationship between EMS personnel and emergency department staff improved vastly. Join opened a new line of communication that not only improved care quality but also positively impacted provider relationships.

A Join client began using MySOS in order to continue collaboration in the post-acute setting. As an Accountable Care Organization (ACO), patient outcomes in acute settings directly impact revenue streams at this center. This application enables medical record sharing and empowers patients to become informed and involved in their treatment process. The goal of this project was to reduce readmissions through improved post-acute care collaboration and information sharing. Reduced readmissions lead to cost savings as well as improved CMS compliance and reimbursement rates for the hospital.

The next stage involved integration with DICOM imaging services, which allows clinicians to seamlessly receive, read, and interpret imaging from their mobile device. The final stage of implementation involved connecting regional EMS services to facilities in the area. Upon completion, this project created a care network that follows acute neurology and cardiology patients throughout the care continuum: enabling increased care coordination and improving patient outcomes.

The collaboration between these MSUs and Join has reduced the time between stroke symptom onset and needle puncture and treatment, improving patient outcomes and reducing morbidity post-discharge. Join has created a line of communication that allows EMS to take on more responsibilities for stroke care in the field and provide more advanced care that is crucial to patient outcomes.

Join supports the fundamentals of clinical research by catalyzing patient recruitment pipelines. The Join platform expands the clinical teams reach in their regional network of care with alert systems established from pre-hospital to post-discharge. Using Join, patient consent & I.D. can begin as soon as EMS arrives at the incident. EMS delivers pre-hospital assessments to notify care teams, CRC’s and clinical investigators of potentially qualified patients, while en route to the hospital. JoinTriage’s early patient assessments help clinical teams identify eligible patients at the beginning of the care continuum while Join’s interoperability with DICOM imaging modalities shortens the read-back time of neurologists by up to 92.5%, reduces the time-to-diagnosis by 40%.
