I have used the Join solution realizing the dual purpose of stroke care and clinical research with its effective team collaboration while working as a Vascular Neurology Fellow. Its simplicity, making clinical data available in real-time, and integration into the large healthcare system where I worked was wonderful for our team of clinicians and researchers. I have been successfully using the Join app over several years for acute stroke care including interventions at comprehensive stroke centers and primary stroke centers as well as remote consulting to triage and transport the patients in the health system and the region across neighboring states for better patient outcomes. The Join medical communications app along with its built-in features of protocol timestamping, telemedicine and DICOM viewer helps with efficient care coordination, collaboration and decision making for time-critical stroke care. Its companion app, JoinTriage, for EMS ambulances is helping first-responders to quickly conduct stroke assessment, identify the appropriate hospital and pre-notify to the destination care team. We’re seeing the crew in transport vehicles becoming an extension of our care team for better preparation and quality care delivery each time 24×7.
The Join app solution is an important advance for providing care for acute stroke patients. It allows physicians and other healthcare personnel to interact with patients and each other remotely in a secure and easily accessible manner.
We have collaborated with Allm to create a custom FAST-ED scale in JoinTriage for West Virginia use by capturing tPA potential and functional status for better patient outcomes. The innovative mobile apps of JoinTriage and Join facilitate the total coordination, real-time communications, team collaboration, sharing information including medical imaging studies, accessing case list, protocol timestamping, telehealth, and seamless clinical research for a better state-wide system of care. A gateway server software is installed on the client environment, taking the qualified images from modalities directly (PACS or VNA), and anonymizes images so no PID (patient identified information) exists. The information is then transferred to the Join cloud on AWS or other cloud partners through a secure VPN connection. DICOM images available on the Join cloud will be securely served to users through streaming rather than direct downloads.

Join is HIPAA compliant and FDA approved. No DICOM images are ever stored on devices, and while DICOM images are instantly viewable, they are never downloaded from the cloud. The app follows all security and privacy best practices, including proper encryption and anonymization of data as well as additional layer authentication on top of OS level authentication.
Once Join is installed in a hospital, an administrator (or multiple administrators) within the hospital is appointed to create and manage groups (also called chat rooms) on Join. Groups are usually created to include all members of a care team for a certain condition or department. The administrator can create groups, add members, and delete members.
To minimize the custom integration costs, Join has a built-in integration feature that allows other applications or web links to launch Join application via HL7 & API’s to share messages with minimum configuration efforts. Additionally, we offer other custom integrations such as developing custom web links which are independent with Join through direct programming configurations or through partner system integrators.

When an emergency department decides to use Join to partner with regional EMS agencies, each ambulance in the area is provided a Join account, as well as training on JoinTriage use. All connections are made by our programmers on the back end, so when an ambulance triages a patient with JoinTriage and sends the information to the hospital through Join, it goes directly to the group message in the Emergency Department and other care team members. The process is seamless, saving precious time and streamlining workflows.
